1) Can I use the diagnostic tool to program Audi, VW, Porsche … besides Benz and BMW?

Answer: Yes, provided you have installed, and downloaded the manufacturer’s programming software, and paid for the valid subscription.
With SmartLink C VCI Box (compatible with J2534-1 & -2; value of 2000+ bucks), you can cost-effectively add new OEM applications for 30+ car brands, with no need to purchase separate, expensive OE scan tools for each brand.

2) What happens if I don't program a new ECU after installing it on my car?

Answer: Nothing will happen, because the system doesn't know how to react, since the PROM that governs the circuits of ECU “what to do” is blank.
It cannot command the injectors to open or the coil(s) to fire. It cannot command the fuel pump to send fuel. It cannot read the external temperature, or even the temperature of the coolant, because there are no instructions defining what is hooked to the ECU, and there is no information to tell the system when to open the injectors, or for how long.
